Purpose and Scope
This policy outlines how employees earn, request, use, and receive payout for paid time off (PTO) at [Organisation Name]. We provide PTO so staff can take predictable, paid time away from work for rest and personal needs.
This policy applies to [all employees / eligible employee groups]. Part-time employees receive prorated amounts as detailed below. Nothing in this policy alters the at-will nature of employment at [Organisation Name].

Responsibilities
- [Role/Management]: Reviews PTO requests promptly based on team coverage and operational needs, explains any declines in writing, and manages schedule coverage.
- [Role/HR or Payroll]: Maintains PTO records, tracks compliance with state and local leave laws, answers balance questions, and conducts periodic policy reviews.

Earning Paid Time Off

Unplanned Absences and Sick Leave
- For unplanned absences or sick leave, notify [Role] by [Time] on the day of the absence via [Phone Number/Communication Channel].
- Provide medical or secondary documentation only when requested by [Organisation Name], following state and local legal limits.
- Absences that follow notification rules and fall within PTO or sick leave guidelines will not count as unscheduled absences under the attendance policy.
Employment Termination
- Balance Payout: Upon termination, accrued, unused PTO is [paid out in the final paycheck / paid out as required by state law / forfeited where permitted by state law].
- Final Employment Date: Employees cannot use PTO to extend their final date of employment without explicit approval from [Role].
